Social Services for Patients in the Primary Care Community
WHO recognised the importance of the social determinants of health which are non-medical factors that accounts for between 30-55% of health outcomes. Family Physicians being the cornerstone of HealthierSG plays an important role in not just managing the disease of the patients but improving the health of Singaporeans by adopting a Biopsychosocial approach which systematically consider the biological, psychological and social factors and their interactions. We will examine how we can work with the community to achieve the aims of HealthierSG and most importantly caring for the individuals in the context of their families, communities and workplace for better health.
We will also highlight practical ways GPs can address non-medical barriers to health by connecting patients with social care.
